The exFAT format will not give any limitations on the file size that you are going to store on your flash drive and you can still use it on both Windows and Macs that are running in Mac OS X Snow Leopard or later version. For you to be able to format your flash drive into Fat32, here is what you need to do. You can also use the FAT32 format on Mac for USB drives if you wish to use it on both Mac and Windows. If you're moving it between Mac and Windows, you want to format your flash drive with the FAT32 filesystem. It shouldn't matter whether you format it on your Mac or on a Windows machine.
To do it on the Mac, plug in the flash drive, and open Disk Utility (in your /Applications/Utilities folder). The drive should appear in the list on the left of the DU window.
Select the drive, then click the Erase tab. On that tab, select MS-DOS (FAT) as the volume format, choose a disk name if you want to, then click Erase, and wait.
That said, if your drive just 'didn't work any more' after a while, it may be faulty and it's possible that re-formatting it won't help. (All flash drives do have a finite number of read/write cycles before they don't work reliably any more, too, although it's unlikely you've reached that limit through normal usage.)

Format an External Disk or a Flash Drive for Mac using Disk Utility in OS X El Capitan and macOS Siera. Formatting an external disk or flash drive in macOS Sierra is little different that formatting it in Mavericks and lower versions of OS X. Disk Utility was changed with the release of OS X El Capitan.
To begin to format USB for Mac and PC on a Windows 10 computer, insert the USB drive into an open port and then launch the Windows Disk Management Utility. To find it, type 'disk management' or 'format disk' in Windows Search and select 'Create and Format Hard Disk Partitions.'
Insert the flash drive or hard drive you want to format for Windows compatibility. Go to the Applications folder on your Mac’s hard drive, then go to the Utilities folder, and launch Disk Utility. Select the drive you want to format. Warning: the following steps will delete any info you currently have on the drive.

NOTE: All SanDisk memory cards and flash drives come pre-formatted and do not need to be formatted out of the box. Memory cards are formatted with the following standards:

SD/microSD	2GB or lower	FAT16
SDHC/microSDHC	4GB to 32GB	FAT32
SDXC/microSDXC	64GB or higher	exFAT
CompactFlash	up to 128GB	FAT32

Reformatting may clear file system corruption and quickly erase everything on the device. If re-formatting is needed, follow the steps below.
WARNING: Backup all your data before formatting. Formatting will erase all data on the memory device.
Formatting your memory device:
1. Double-click My Computer, or Computer (for Vista and Win7)
2. Right-click the drive letter associated with your memory device, then select Format

WARNING: Make sure you select the correct drive letter.
3. Select a File system type.
- FAT
- FAT32
- exFAT

4. Ensure 'Quick Format' is checked
5. Click Start.
NOTE: exFAT is supported natively in WIN7 and WIN8. You will need to download a patch for Windows XP or VISTA to format a 64GB or larger device.
